New York Certificate of Incorporation Not-For-Profit Corporation | Form DOS 1511-f

The New York Certificate of Incorporation Not-For-Profit Corporation | Form DOS 1511-f is a template form which must be filled out and submitted with all required documents and payment to the New York State Department, Division of Corporations when a corporation is ready to apply for a New York Certificate of Incorporation. A successful filing with this paperwork with this governing entity does not grant Tax-Exempt Status. Thus, it is crucial that Incorporators contact the Internal Revenue Service before filling out this form should it intend to apply for such status in the future.

The New York Certificate of Incorporation Not-For-Profit Corporation may be submitted by mail, in person, or by fax. This will need to be accompanied with a payment for the Filing Fee.
